How do I check if a service requires prior authorization?
To check on a specific code or service, please call our Support Team at (866) HEART-US (432-7887). Providers can use this same phone number to request approval and check the status of an existing approval.
For a list of services that require prior authorization, please refer to page 13 and 14 of the Decent Provider Manual. To request authorization for a specific service, please fill out an Authorization Request and email it to email@example.com or fax it to (512) 729-7178.
