What you need for a health insurance quote.

A census and a few company details help Decent find viable plans and compare their prices. If you already offer coverage, your latest renewal gives us a starting point, too.

Start with what you already have.

Your current broker, payroll or benefits system, and business records may already hold most of the details. Together, they help Decent check which plans fit your team and compare the quotes that come back.

If you have a plan

Look for the latest renewal packet or plan summary in your broker's emails or carrier portal. It provides a baseline for coverage, price and renewal timing, so you can compare new quotes with what you have now.

If you are starting from scratch

Your payroll or HR system is a starting point for the employee roster. Confirm who may enroll, any dependents and who will waive coverage. Your business records have your legal name, EIN and headquarters address.

What goes into a quote

The details below help determine who needs coverage, where plans are available and when coverage should begin. Requirements vary by insurer and plan type.

Copy the census template

Employee census

Start with your payroll or benefits roster, then check dependent and waiver details. Ages and home ZIP codes help price coverage and check which plans are available where your team lives.

  • Full name, date of birth, gender and home ZIP code for every employee.
  • The same for any spouse or children they would cover.
  • Names of anyone waiving, and whether they have coverage elsewhere.

Company information

Check tax and company records for your legal name, EIN and headquarters address. Your current renewal, if you have one, helps you choose a target start date. Those details help quote the right company and coverage period.

  • Legal name and EIN.
  • Company HQ address.
  • When you want coverage to start.

Why quote timing varies

Complete details make it easier to rule out routes that do not fit and compare the options that do. Some rates come from filed or public prices; others need carrier review and more information before a quote is available.

Filed-rate routes

Fully insured, ICHRA and QSEHRA options may be priced sooner where eligible.

Underwritten routes

PEO, level-funded and self-funded options need further review and may take one to two weeks to quote.
